Product Theater: Diminishing Perspectives: An Exploration of the Challenges Faced by a Patient With Heart Failure

Speaker: Robin J Trupp, RN, PhD, ACNP-BC, CHFN, FAHA
Adjunct Assistant Clinical Professor
University of Illinois at Chicago, School of Nursing

Program Description:
Please join us for a detailed discussion focusing on a patient and his family, as they grapple with challenges in our new film “Diminishing Perspectives.” Patients suffering from heart failure and their families face a multitude of challenges on a daily basis, not only with the management of multiple medications, but also with attempting to incorporate new lifestyle changes into their daily routine. Following hospitalization for heart failure, patients often face uncertainty regarding their ability to resume a normal life. Considerable opportunity exists for heart failure nurses to assist patients in overcoming challenges and achieving guideline and dietary compliance.

Program Objectives:

Understand the various challenges faced by patients with heart failure
Identify opportunities for heart failure nurses to assist patients in overcoming these challenges and achieving guideline and dietary compliance

2. Explain the rationale for measuring & utilizing pulmonary artery pressure to guide patient therapy.
3. Identify patient characteristics and clinical outcomes reported in the CHAMPION clinical trial.
4. Utilize changes in heart failure diagnostic data to predict clinical events.
5. Identify barriers in clinical practice to the practical integration of device data.
6. Integrate device data into the everyday management of heart failure patients.
7. Provide appropriate care and counsel for heart failure patients and their families.

There is a Heart Failure (HF) epidemic going on worldwide and nationally. It is one of the few
cardiovascular disorders on the rise. According to the World Health Organization (WHO), there are over 23 million cases of HF worldwide and 6 million in the US alone. In the US, there are an estimated  670,000 new cases of HF diagnosed annually. This condition is a major cause of morbidity and mortality. It is the leading cause of hospitalizations of the elderly in the US. There are over 1 million hospitalizations annually and over 50% of these patients are readmitted within 6 months of discharge.

Heart failure is the single largest expense for Medicare costing over $32 billion annually.
Despite the risks of cardiovascular disease, including heart failure in African Americans, some guideline indicated therapies are disproportionately underused in this population. Heart failure studies have demonstrated that treatment guidelines are adopted slowly, are applied inconsistently, and as a result, often fail to lead to improvements in patient care and outcomes. ACC/AHA practice guidelines give a class 1 recommendation for the use of H-ISDN in self-identified black patients with symptomatic systolic heart failure on optimal therapy. In the 2010 HFSA practice guidelines, a combination of hydralazine and isosorbide dinitrate was recommended as part of standard therapy in addition to beta blockers and ACE inhibitors for African Americans with HF and reduced LVET. Yet, a recent study indicated that only 7.3% of eligible black patients are actually receiving H-ISDN. A registry of hospitalized HF patients showed only 4.5% of blacks and 2.6% of whites went home on I-SDN. Establishing educational initiatives should help to reduce practice variability, eliminate gaps between guidelines and practice, and improve the outcome of patients with HF.

Speakers: Jennifer Keeley, CRNP, DNP
Nurse Practitioner
Comprehensive Pulmonary Hypertension Clinic
University of Pittsburgh Medical Center
Pittsburgh, PA

Description: Management of patients with PAH requires a holistic, multidisciplinary effort. Nurses working in the fields of pulmonology, cardiology, rheumatology, and intensive care should have a clear understanding of the diagnostic criteria, quality-of-life issues, and medical management considerations for patients diagnosed with PAH. Issues for nurses include early identification and referral of patients with symptoms suggestive of PAH for appropriate diagnostic testing, understanding the risk-benefit equation for specific PAH therapies and associated treatments, and nursing methods of support in crucial clinical management roles to improve patient quality of life and medical outcomes.
