Pre-Conference Workshop: Heart Failure Disease Management Program

This year, AAHFN's Heart Failure Disease Management workshop is titled "Kicking it up a Notch". This workshop will build on basic concepts that have been presented in past years. We will focus on specific innovative strategies to manage high-risk patients in a disease management program without incurring a traditional hospital admission. We have set aside time for selected participants to share their innovative programs or strategies that have been successful in managing high risk heart failure patients.  We will email everyone registered for the workshop in May and ask interested participants to submit a one page abstract outlining their program's innovation. If accepted for presentation, the participant will be asked to prepare 3-5 slides to be submitted in advance of the workshop.
 
This one-day workshop will offer cutting-edge ideas for nurses working in a heart failure disease management program and an opportunity to network with other heart failure nurses who participate in heart failure disease management programs across the United States. This is an opportunity for sharing and learning new information to take your heart failure program to the next level. The target audience is staff nurses, advanced practice nurses, and even administrators who are looking for creative ideas to implement.
