Heart Failure Disease Management Program

This workshop will deliver evidence-based information on the essentials of developing and managing inpatient and outpatient heart failure disease management programs (HFDMP).

Topics will include: How to get started, How to market your program, Putting together the right team, Exploring practice models, Inpatient practices to improve efficiency of care, Systems of care to reduce readmissions, Best ways to educate your patients, Getting your program certified, Measuring outcomes, and Ideas to take your program to the next level.

A panel of experienced HF nurses will share information and answer questions from the audience during an interactive session designed to help audience members frame ideas for their own program. All attendees will receive tools and information necessary for implementing a heart failure program at a local level or for enhancing an existing heart failure program.
